When calculating the mean-adjusted chi-square per the formulas in appendix 5, which is the appropriate df to use - the d(0)-d(1) of the more and less restrictive models being compared or the scale-factor adjusted df, denoted c(d) in the appendix?

The degrees of freedom would be d0 - d1. You might find the writeup that can be accessed from the homepage more helpful. You would start at step 3 because you will obtain the scaling correction factors from the outputs.
